In order to qualify you must meet ALL three criteria below:

1. Be a current member of CHEA of California (please supply your CHEA membership number with expiration date).
2. Be a current member of HSLDA (please supply your HSLDA membership number with expiration date).
3. Contribute at least $40 per year to Family Protection Ministries.
