PSP Sports Registration Form
This form is used for all of our events EXCEPT Animal Care Class.
You may print this form, complete it (two pages), and turn it in to PSP Sports and More (see our contact information HERE).
In order to be registered you must submit payment with your registration form. If you are registering in person you may pay by cash, check, or credit card. We are also accepting mail-in registrations, by check only, if postmarked by August 22, 2018; see our Contact Us page for mailing information.
